Botox is administered by our Board Certified MD, Dr. Robert Simon and our Board Certified Physician Associate, Carrie. $12 per unit. *If you need to reschedule, we ask for a 24 hour advance notice. *Failure to comply with cancellation policy will result in non-refundable fees- $25 for appointments rescheduled <24hours and $50 for NO SHOW appointments. **$50 commitment fee is applied to treatment.**
